You are responsible for providing us with current insurance information and remembering your co-pay. Payment is expected at the time of service unless prior arrangements are made with our billing department, and may be made by cash, check or credit card. On accounts which have established payment arrangements, the payment is due upon receipt of the monthly statement. In lieu of payment at the time of service, we will gladly bill the following insurers or insurance plans directly:

First Choice Health Plan
Group Health Options & Options PPO & Cooperative
Blue Shield (Blue Card) Plans
WA Medicaid (DSHS Public Assistance)
United Healthcare Healthy Options
Annual exams may or may not be covered by your insurance policy. It is up to you to verify and understand your coverage with your plan prior to being seen, and provide us with all the information necessary to bill your insurance company. We do not guarantee payment. You will be responsible for payment of all services not covered by your insurance company. Known deductibles not yet satisfied are also due at time of service.
Our bookkeeping staff is available to accept credit card payments over the phone and to discuss any financial questions you may have.
